Moriarty was releases by Sherlocks brother from prison as they couldn't get them to tell them a code that could hack anything. Sherlock was setup by moriarty to look like a fraud, Moriarty catches Sherlock out when they meet on the roof of a hospital he tells him that he has hired a gang of murders that have been told to kill all of his loved people and the people he is associated with.and if he doesn't jump of the building. Sherlock starts to do what he does best tries to get in Moriarty head to get the phone numbers for the murders to call them off. Moriarty shoots himself in the head. Sherlock makes John belive Moriarty then jumps. as John runs to Sherlocks body he is knocked over by a cyclist. Sherlock is in a pool of blood and is taken away on a hospital bed. Last scene John is talking to Sherlocks grave then it shows John from another angle then zooms out to show Sherlock watching John.

how i think he survived:
as Sherlock jumped there was a rubbish van in front of him i think he landed in that then jumped to the floor. in the previous episode they dealt with a drug that caused hallucination i think the cyclist drugged John as he was knocked over.
